I'm answering this now in case other people have a similar question. It turned out to be that the debt was sold to another collection agency. The first one removed their delinquency from my report, and the second one gave me a chance to pay it before they reported. I settled it in that time, and if they keep their word (they gain nothing by reporting it now anyway) it won't be showing back up.
Yes, any negative accounts, especially collections, can be re-inserted on your reports. I heard its uncommon, but twice now I've had 6-7 medical collections removed then returned a few months later. However, most collection agencies don't always follow the laws when reporting accounts to the credit bureaus, so research as much as you can to learn your rights. If not they'll take advantage of you.
